At AFC Urgent Care, we provide immigration physicals in Agoura Hills, CA. Our Immigration Medical Exams (Green Card Medicals) are offered to applicants in Agoura Hills, Thousand Oaks, Woodland Hills, Calabasas, and the surrounding areas. A required step in many visa and Green Card applications, these physicals must be completed by USCIS-designated Civil Surgeons — a certification our doctors proudly hold.

We speak Spanish, Russian, Arabic, and have translation services available for other languages.

Our 3-Step Process for Immigration Physicals

We follow a structured three-step process to complete your immigration medical exam:

Step 1: Pre-Screening and Vaccination Review

Before visiting our clinic, please email us at agourahillsteam@afcurgentcare.com with the following:

  • Your full name and date of birth
  • Your vaccination records (translated into English if necessary)
  • Your contact information (phone and email)

Our staff will review your vaccine records to ensure they meet CDC immigration requirements. If any vaccines are missing, we will inform you.

Vaccines Available In-House

  • Tdap – $100
  • Polio – $95
  • Flu – $35 ($65 for patients 65+)

Note: Flu shots are required during the flu season only (October 1 through March 31). You may also receive vaccines from your primary care physician or pharmacy, and email us proof of immunization.

Titers Testing (If Vaccine Records Are Unavailable)

If you no longer have access to previous vaccination records, titers (blood tests that prove immunity) are an option. We offer MMR, Hepatitis B, and Varicella titers for $150 total (insurance does not cover this).

Please note that if your vaccine records are in another language, you must have them translated into English by a certified translator. The translated records must be accompanied by a certificate/certified statement of translation. Once we verify that you meet vaccination requirements, you can proceed to Step 2.

Step 2: Immigration Bloodwork and Payment

Cost of the physical exam with completiton on the form I-693 including testing (vaccines are not included) is $400 per person. Once your vaccines are confirmed, you may walk in at your convenience for the required testing — no appointment is needed.

  • Syphilis Test
  • Gonorrhea Test

Results: Typically available in two to four business days.

Once your lab results are in, we will call you to schedule your physical with our Civil Surgeon.

Step 3: Physical Exam with Civil Surgeon

Dr. Adel Jabour, a USCIS-designated Civil Surgeon, will conduct your immigration physical exam in Agoura Hills, CA. This exam is offered on Wednesday afternoons by appointment only; exam duration is 20 to 30 minutes.

  • Full-body examination
  • Final verification of vaccinations and lab results

Upon completion, you will receive:

  • A signed and sealed envelope containing your I-693 form (for USCIS submission)
  • A copy of the I-693 for your personal records

  • Hospital/clinic records (including past treatment for TB or infectious diseases)
  • Prior chest X-rays or diagnostic test results
  • Mental health records (if applicable)
  • Form I-693 (You may complete it online or at our clinic; it must be filled out in black ink and capitalized)
  • Required Form I-693
  • Payment ($400, cash or credit/debit; no checks accepted)
