Using HealthMark’s HIPPA-compliant portal, you can quickly request a copy of your medical records 24/7. You can receive a copy of your medical records directly through the HealthMark patient portal, with the option to have your records delivered electronically to your email, via fax or through the mail.

By taking this process online, patients now have better access to their medical records without the hassle of having to contact their care provider. You can submit an update to your medical records or request that a copy of your medical records be sent to you by following these instructions. Once logged in, select “Submit Request” from the menu
options and enter all required fields to provide authorization directly to HealthMark
Insurance Chart Requests and Legal Chart Requests
Patient Charts can be requested through HealthMark’s Request Manager (https://requestmanager.healthmark-group.com) for compliance chart reviews. The same is true for Legal Chart Requests. Just follow the instructions on HealthMark’s Request Manager to get copies of the requested charts.
