To save time, we recommend that drivers fill out the health history questionnaire at home. To make sure the exam can be completed within the same day, drivers with certain medical issues need to bring certain documents depending on any applicable health conditions.

In order to be best prepared for the examination, please bring the following:

  • Your valid driver’s license / identification
  • Eyeglasses, contact lenses, and hearing aids if applicable
  • A list of any current or past medications
  • A record of your blood sugar readings and your most recent Hgb A1C if you have diabetes
  • A record from your CPAP machine showing proper use for at least 90 days if you have sleep apnea
  • A letter from your cardiologist indicating you're safe to drive a DOT physical if you have heart issues.

What Happens During a DOT Physical

During your DOT Physical, a medical provider will start by reviewing your medical history with you. Typically, you will be asked questions about your medications, past surgeries, health habits- including drinking or smoking, and any other health concerns you may have. These questions are designed to help your provider better understand your current health and whether or not you’re fit to drive commercially. A DOT physical also includes a physical examination that covers your vision, hearing, blood pressure, and urinalysis. A urine screen is necessary to check for general health levels and underlying medical conditions, like diabetes. The physical exam scans for any potential concerns that could put you at risk for not being healthy enough to operate a commercial vehicle without risk of injury to yourself or others.
