What Are The Requirements to Pass a DOT Physical?

A Department of Transportation (DOT) physical is a medical exam that is required for all commercial drivers to obtain their commercial driver's licenses. If you’re a commercial driver, or plan on becoming one in the future, a DOT physical is required by law to ensure safety for you and those around you.

DOT Physical Requirements

To best prepare for your DOT physical, it’s helpful to know what your healthcare provider will be testing for so you can obtain your commercial driver’s license (CDL).

  • To start, your provider will conduct a review of your medical history, asking you for a complete list of medications you’re currently taking. Certain medications that impairs your focus and compromises your safety may disqualify you from passing the physical requirements.
  • The examiner will also lead you through a physical examination, checking your general appearance, spine, ears, mouth and throat, heart, abdomen, reflexes and coordination.
  • Also, be prepared to take a vision test, as driving a commercial vehicle requires that you have proficient visual acuity. To pass the visual exam, you need to have at least 70° horizontal peripheral vision in each eye and at least 20/40 vision with or without using corrective lenses.

The tests associated with a DOT physical exam vary case-to-case, depending on your overall health and wellness. DOT medical cards are valid for up to 2 years if you meet all the FMCSA DOT regulations for commercial driver fitness.

What do I need to bring to the exam?

If you wish to save time during the appointment, you can fill out the driver's portion of the exam at home. The form can be found here. Regardless, any necessary paperwork will be provided by the clinic. Try not to use the restroom just before your appointment, as you will likely need to give a small urine sample for a kidney screen.

In order to be best prepared for the examination, you should bring the following:

  • Your valid driver’s license and /or other photo ID.
  • Eyeglasses, contact lenses, and /or hearing aids if applicable.
  • A list of medications, and the name and contact information of the prescribing doctor.
  • A record of your blood sugar readings and your most recent Hgb A1C if you have diabetes.
  • A record from your CPAP machine showing proper use for at least 90 days if you have sleep apnea.
  • A letter from your cardiologist indicating you're safe to drive if you have heart issues.
