With the new school year slowly creeping up on us, it’s time to start thinking about physicals for the Fall season! Scheduling an annual physical is important for a child’s health, especially since their bodies are constantly developing.
School physicals allow parents, teachers and doctors to monitor a child’s growth and health. In addition, school physicals can help prevent serious diseases and old injuries from reoccurring, detect previously unaware conditions and ensure good health.
You may be saying, “Getting a physical sounds like a great idea, but what do I need in order to get one?” If you are unable to get an appointment with you primary care physician, you can always stop in at AFC Urgent Care. These are the materials needed to obtain a physical at the center:
- School/Work/Sport physical form
- Vaccination records of the patient
- Photo ID of the guardian or patient if over the age of 18 and an insurance card*
At AFC Urgent Care Fairfield, we can perform you and/or your child’s sports or school physicals on site by our board certified physicians. We are open every day from 8 am – 8 pm, and located on 1918 Black Rock Turnpike. You can also reach us by calling (203) 583 – 8400.
