We understand and recognize the significance of immigration physicals as an essential step for individuals seeking permanent residency. Our USCIS-authorized civil surgeon provides immigration physicals and necessary documentation in compliance with the USCIS requirements.
Please call AFC Urgent Care Frazer to set up an appointment at our clinic. When you schedule your appointment, you will be asked to pay for the exam portion of your visit. If you need to cancel or reschedule, please give us 24-hour notice and we will refund your payment in full. We have a limited number of spots for immigration physicals each week and many patients needing services. If you do not cancel with a 24-hour notice, we cannot fill your spot, and you will forfeit the amount you pre-paid.
Individuals applying for citizenship or seeking Permanent Residency (Also known as a Green Card) are required to obtain an immigration physical from a USCIS Designated Civil Surgeon. It is mandatory that immigration physicals be completed by a Designated Civil Surgeon in order for the exam to be valid. We are pleased to be able to provide this service to individuals needing this exam. We do all the necessary testing on site and have all of the required vaccines in our office.
AFC Urgent Care Frazer can perform the required immigration physicals in accordance with guidelines provided by the Department of Homeland Security, U.S. Citizen and Immigration Services, and Centers for Disease Control for immigrants or employers who hire employees originating from another country. The civil surgeon will record the exam results on Form I-693 for the applicant to return to the USCIS (INS). At AFC Urgent Care Frazer, Immigration physicals are available by appointment only.
The immigration process can be overwhelming and stressful, which is why we prioritize providing a supportive and welcoming environment for our patients. Our staff is friendly, knowledgeable, and committed to addressing any questions or concerns.
We perform every service you will need to pass the exam, and includes the following:
Do not receive any additional vaccines until consulting with our civil surgeon. Strict new guidelines regarding tuberculosis skin testing affect when you should receive vaccinations.
After The Exam: Once the immigration physical examination is completed, AFC Urgent Care Frazer will certify Form I-693 with the test results of the immigration exam and vaccination history. Please present this sealed envelope to the Department of Homeland Security and U.S. Citizen and Immigration Services with your immigration application. You will also be provided with a copy of these forms. Please allow at least 1 week for the completion of these forms. We will contact you when they are ready to be picked up.
Your doctor will order a chest X-ray and blood tests. Your physical and mental disposition will be assessed through X-rays, blood tests, and diagnostic observation. In addition to your Form I-693, bring your medical and vaccination records for the doctor. If you don’t have proof of your vaccinations, you can request a blood test to prove immunization.
