AFC Urgent Care of Portland can perform immigration physicals in accordance with the requirements of Department of Homeland Security (DHS) and U.S. Citizen and Immigration Services (USCIS) for immigrants from another country. Each evaluation includes an examination performed according to the medical guidelines established by the DHS, USCIS and the Centers for Disease Control (CDC). Our examining civil surgeon will record your exam results on Form I-693 for you to return to the USCIS (INS).

Our USCIS Civil Surgeons are experienced with the medical requirements of immigration physicals, and are able to provide you with everything needed to complete the medical part of your immigration application. This includes the physical examination, the blood tests, chest x-rays and any vaccinations if needed. We also accept proof of vaccination done elsewhere to help you complete your exam faster if possible.

Schedule Your Immigration Physical

Expectations

When you come to your appointment, please arrive 30 minutes early for paperwork to be filled out. You will have a minimum of two visits to complete your immigration physical. The first visit will be with a medical assistant to review your paperwork and initiate some of the required testing. During this visit, the Medical Assistant will collect all the necessary documentation and record it on your I-693 form.  The second visit will be with your Civil Surgeon to complete your physical and obtain any necessary immunizations. Additional visits may be necessary depending on the Civil Surgeon schedule and test results. The second visit for the Civil Surgeon exam will be scheduled about 7 days following the initial visit. This is to allow sufficient time for labs to come back.

During the exam, the Civil Surgeon will review health history and conduct a full body examination. (For females, a female staff member will be present in the room if there is a male Civil Surgeon). Once health history is reviewed with the Civil Surgeon, he or she will inform the Medical Assistant to give any further vaccinations, immunizations or blood draws.

Once all reports have been received and reviewed, you will be notified that the documents are ready to be signed.

Included in the Immigration Physical examination:

  • Physical examination and completion of all the necessary paperwork
  • Physical and mental evaluation
  • A review of all available medical records
  • Evaluation of other physical or mental abnormality, disease or disability
  • Vaccinations and status verification
  • Chest x-ray (if needed)

Administration of vaccines, and any other required services are available at additional cost.

What you need to bring

Patients are advised to bring an interpreter with them if needed. We can provide an interpreter with two days advance notice. If we provide the interpreter, there is a fee of $125.

Patients are advised to bring the following to their appointment:

      • Government issued picture ID such as a driver’s license or passport (if physical is for a child, the child will need photo ID as well. Parent ID will not be accepted)
      • Vaccine/Immunization records (if records are in a different language it will need to be translated into English): www.uscis.gov/i-693
      • Proof of COVID vaccination must be brought to appointment
        • Patients must be fully vaccinated before the form is released
        • AFC does not provide COVID-19 vaccinations
      • Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious diseases
      • Prior chest x-rays and/or other diagnostic tests
      • Any mental health records
      • We kindly request USCIS visitors to bring all correspondence from the USCIS with them.
      • Payment (we accept cash or credit).
