How Do I Prepare For the Exam?
If you wish to save time during the appointment, you can fill out the driver's portion of the exam at home. The form can be found here. Otherwise, all necessary paperwork will be provided by your healthcare provider. Upon arrival to the physical, wear loose, comfortable clothing. Don’t use the restroom just before your appointment, as you will need to give a small urine sample for a kidney screen.
In order to be best prepared for the examination and any questions your provider may ask, please bring the following:
- Your valid driver’s license and /or other photo ID.
- Eyeglasses, contact lenses, and /or hearing aids if applicable.
- A list of medications, and the name and contact information of the prescribing doctor.
- A record of your blood sugar readings and your most recent Hgb A1C if you have diabetes.
- A record from your CPAP machine showing proper use for at least 90 days if you have sleep apnea.
- A letter from your cardiologist indicating you're safe to drive a DOT physical if you have heart issues.

What to Expect During the Examination:
