AFC Urgent Care Mentor can perform immigration physicals in accordance with the requirements of Department of Homeland Security (DHS) and U.S. Citizen and Immigration Services (USCIS) for immigrants from another country. Each evaluation includes an examination performed according to the medical guidelines established by the DHS, USCIS and the Centers for Disease Control (CDC). Our examining civil surgeon will record your exam results on Form I-693 for you to return to the USCIS (INS).
Our USCIS Civil Surgeons are experienced with the medical requirements of immigration physicals, and are able to provide you with everything needed to complete the medical part of your immigration application. This includes the physical examination, the blood tests, chest x-rays and any vaccinations if needed. We also accept proof of vaccination done elsewhere to help you complete your exam faster if possible.
If you’re seeking Permanent Residency or a Green Card, and are required to obtain an immigration physical prior to approval of your application, schedule your immigration physical today.
The United States Citizenship and Immigration Services (USCIS) requires you to get a full immigration physical exam. This exam is to make sure you don’t have physical issues or mental disorders that may prevent you from being approved. Our physicians understand that USCIC doctors must follow the government’s strict guidelines and procedures for your exams. You can rest assured that your forms and report will be done correctly.
Not all providers can perform immigration exams. Only physicians designated as civil surgeons by USCIS can complete an immigration physical, and AFC Urgent Care Mentor has Dr. Cingireddi Laxman MD as the doctor doing immigration physicals.
At AFC Urgent Care Mentor, your certified USCIS civil surgeon will order a chest X-ray and blood tests. Your physical and mental disposition will be assessed through X-rays, blood tests, and diagnostic observation.
Unlike a regular physical exam, immigration physicals are used mainly as a screening tool to check whether it is safe to allow a person into the United States. Certain medical conditions may require you to be placed in quarantine until you get better. Other conditions can bar you from admission to the United States, such as serious communicable diseases posing a public health threat.
When you come to your appointment, please arrive 30 minutes early for paperwork to be filled out. You will have a minimum of two visits to complete your immigration physical. The first visit will be with a medical assistant to review your paperwork and initiate some of the required testing. During this visit, the Medical Assistant will collect all the necessary documentation and record it on your I-693 form. The second visit will be with your Civil Surgeon to complete your physical and obtain any necessary immunizations. Additional visits may be necessary depending on the Civil Surgeon schedule and test results. The second visit for the Civil Surgeon exam will be scheduled about 10 days following the initial visit. This is to allow sufficient time for labs to come back.
During the exam, the Civil Surgeon will review health history and conduct a full body examination. (For females, a female staff member will be present in the room if there is a male Civil Surgeon). Once health history is reviewed with the Civil Surgeon, he or she will inform the Medical Assistant to give any further vaccinations, immunizations or blood draws.
Once all reports have been received and reviewed, you will be notified that the documents are ready to be signed.
Administration of vaccines, and any other required services are available at additional cost.
Patients are advised to bring the following to their appointment:
AFC offers some of the lowest wait times around, with many patients getting in to see a provider in less than 15 minutes. Your exam will probably take around a half hour.
We’ll complete your paperwork as required, but if it’s something we can help with we might be able to provide you with treatment on the spot. With in-house labs at many locations, you could be on your way without the need for a second visit.
You can stop into any AFC, any time. Check out our list of locations to find the one nearest you.
