PATIENT BILL OF RIGHTS AND RESPONSBILITIES



Patient Rights Include:

  • Right to be treated with courtesy, respect, and dignity
  • Right to privacy
  • Right to a prompt and reasonable response to questions and requests
  • Right to receive accurate and easily understood information about their care plan (diagnosis, treatment, alternatives, risks, and prognosis).
  • Right to participate in decisions regarding their care
  • Right to refuse care to the extent permitted by law and to know the consequences of refusing.
  • Right to submit a complaint or grievance for follow-up by AFC
  • Right to refuse the release of Personal Health Information (except when permitted by law)
  • Right to access their medical records and have them amended it needed
  • Right to a language interpreter or sign language or drawings to communicate
  • Right to receive, upon request, a reasonable estimate of charges for medical care.
  • Right to impartial access to care, regardless of race, creed, sex, sexual orientation, age, national origin, physical disability, or source of payment.

Patient Responsibilities Include:

  • Responsible for providing accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his or her health.
  • Responsible for notifying provider if you do not understand your diagnosis, treatment or prognosis
  • Responsible for notifying provider about any limitations or barriers to proposed care plan
  • Responsible for following the provider’s prescribed plan of treatment, care and services.
  • Responsible for reporting unexpected changes in their condition to the provider.
  • Responsible for their actions if they refuse treatment or do not follow provider’s instructions
  • Responsible for being considerate of other patient and personnel ‘s rights, privacy, and property, and in assisting with noise control, not smoking, limit visitors with you, or waiting in car if needed.
  • Responsible for fulfilling your financial obligations associated with your health care.
  • Responsible for safeguarding any valuables or personal belongings.
  • Responsible for understanding your medical insurance plan and obligations (deductibles, etc).
