At AFC Urgent Care Port Chester, we provide complete Immigration Physicals for individuals seeking U.S. residency. Our certified civil surgeons follow the U.S. Citizenship and Immigration Services (USCIS) guidelines to ensure a seamless and compliant process.

What’s Included in Your Immigration Physical:

1. Medical Examination by a USCIS-Certified Civil Surgeon

2. Review of Vaccination Records – Ensure you meet the vaccination requirements. If vaccinations are missing, we can administer them.

3. Required Lab Tests:

• Tuberculosis (TB) testing
• Syphilis blood test (for individuals 14 years and older)
• Gonorrhea test (for individuals 14 years and older)

4. Completion of Form I-693 – Your immigration paperwork will be completed and sealed for submission to USCIS.

Required Documents:

• Government-issued ID (Passport, Driver’s License, or Green Card)
• Vaccination Records – Bring a copy of all previous vaccinations.
• Form I-693 – You can download this from the USCIS website or fill it out at our office.

Pricing:
