At AFC Urgent Care in Princess Anne, we are pleased to offer immigration physicals. If you are immigrating—applying for a green card or permanent residence—to the United States, an immigration physical is a necessary step in the immigration process. This physical examination is performed to make sure that you are medically and physically in good health when you arrive to the United States, and it also prevents diseases from entering the country.
Who Performs The Immigration Physical?
These exams can only be performed by a designated civil surgeon, or a doctor who has received certification by The United States Citizenship and Immigration Services (USCIS). At AFC Urgent Care in Suffolk, VA, we are pleased that board certified physician Dr. Clarke is a civil surgeon and is able to provide immigration exams to our patients at our AFC Princess Anne.
What To Expect At An Immigration Physical
The physical involves both a physical and medical exam. Dr. Clarke will discuss your medical history and your family’s medical history, and look for any health conditions. You should also be prepared to talk about your vaccination records. However, if you do not have your vaccination records, we will perform a blood test. During your appointment, we have the following vaccines and tests available. However, we recommend calling ahead to make sure that the following will still be available during your appointment.

What To Bring To Your Immigration Physical
When you arrive for your appointment, we request that you please bring the following with you:
- Pre-completed I-693 Form (Report of Medical Examination and Vaccination Record)
- Valid driver’s license or passport
- Vaccination or immunization records
At the end of your exam, Dr. Clarke will fill out the rest of the 1-693 form and place it in a sealed envelope for you to send to USCIS. We will also provide you with a copy of the form for your records. Please note that USCIS will not accept the envelope if it has been opened.
