I-693 MEDICAL EXAMS AVAILABLE

In order to receive immigration physical exam completion and to fill out the I-693 form, you must undergo a medical examination by a certified civil surgeon, completion of USCIS required immunizations, completion of required blood and urine testing, completion of tuberculosis testing, and filling your I-693 paperwork.

    • Medical exams are a requirement for all applicants; all green card physical examinations consist of general health checks including temperature, pulse, heart, blood pressure, eyes, and ears. Your civil surgeon will review medical records pertaining to previous vaccinations, tuberculosis tests, and other medical records.
    • A USCIS Physical exam requires medical surgeons to verify all of your previous vaccinations. Some vaccines are required by the Immigration and Nationality Act, while the Center for Disease Control and Prevention (CDC) requires others. For permanent resident status, you must receive the following vaccines:
      • Mumps, measles, and rubella
      • Hepatitis A and B
      • Polio
      • Varicella
      • Tetanus and diphtheria toxoids
      • Influenza
      • Meningococcal
      • Pneumococcal pneumonia
      • Haemophiles influenza type B
      • Pertussis
    • If already vaccinated, bring your vaccination reports to your doctor. If you haven’t received your vaccinations, the civil surgeon will help administer them.
    • Applicants 15 and older must undergo syphilis and gonorrhea testing. Some applicants may not have symptoms of the diseases; therefore, testing must occur to ensure no one becomes infected in the US. Testing occurs through a RPR blood test.
      • If the test results come back normal for syphilis, you’ve completed the examination. If you receive abnormal results, the laboratory will run a second test for any other abnormalities. If the T. Pallidum-PA test comes back abnormal, the patient must be treated for syphilis before finishing the examination forms.
      • If patients have a normal gonorrhea urine test result, they pass examination. If test results come back positive, applicants must receive treatment with proper medication.

    Patients must bring valid government issued IDs, including a passport, US driver’s license, or a state ID. Patients must also provide proof of other vaccinations, copies of medical history records, and copies of chest x-rays, if any.
