American Family Care offers a number of patient resources, including the following:
pre-registration forms, statement inquiry, or medical records release form.
American Family Care accepts most insurance plans. You may call the Member services on the back
of your card to confirm we are in network. We will verify all insurances during your check-in
process. We must have photo-copy of the front and back of each insurance card you want to use
for your visit at the time of visit. Without this information, we cannot bill your
insurance and your visit will be self-pay at the time of service. All Payments are expected at
the time of services including Copays, Deductible payments, non-covered services, and self-pay
charges. These payments will be collected during Check-In.
AFC can also file your claim for you; however, any co-pays, deductibles, or non-covered services
must be paid at time of services rendered. We also accept Cash, Check, Visa, Mastercard,
Discover Card, and American Express. Please be advised, effective 01/15/2024, that we will begin
charging a 2.90% surcharge fee to all charges paid by Credit Card. This is our effort to offset
the rising Credit Card processing fees. However, to avoid this extra fee, we do accept cash
payments as well.
As a convenience to our patients, we do file with most insurances companies for you. We also keep
your credit card securely on file for future balances, such as any unpaid balances after your
insurance has processed your claim. We will then charge you credit card on file within one week.
You will receive text and email message regarding any balances prior to your charge being card.
