The United States Citizenship and Immigration Services (USCIS) requires medical examinations on all individuals applying for U.S. green cards or citizenship. Physicians designated as Civil Surgeons by are authorized to conduct immigration physicals. AFC Urgent Care San Diego has qualified Civil Surgeons on staff able to perform immigration physicals for applicants of all ages.
Immigration physicals are used as a screening tool to check whether it is safe to allow a person into the United States. Some medical conditions may require you to be placed in quarantine until you get better while other more serious communicable diseases which left untreated could pose a public health threat and inhibit your admission into the United States.
Medical services needed to pass the exam:
Physical Examination and completion of all the necessary paperwork
QuantiFERON with Interpretation for tuberculosis (TB).
Government issued photo ID such as a driver's license or passport
A copy of any prior immunization/vaccinations/shot records translated into English
Prior diagnostic test
Any mental health records
A completed version of the most recent Immigration Physical Form I-693

If you do not have your vaccination records, we can perform a blood test for a fee to prove you have been previously immunized. AFC Urgent Care San Diego also offers the vaccines listed below for an added cost. Any additional required vaccines, tests, or x-rays will be based on the Physician’s recommendations, Center for Disease Control (CDC) guidelines and patients’ age:

After The Exam:
Once the immigration physical examination is completed, AFC Urgent Care San Diego will certify Form I-693 with the test results and provide you with a copy. Please present this sealed envelope to Department of Homeland Security and U.S. Citizen and Immigration Services with your immigration application for processing.
