Before Your Visit

  • Schedule your appointment online or by calling (720) 502-0466.
  • Complete the I-693 form electronically after registering online. This will save you time at your first visit. Once you register, you will receive an email with a link to an online form.
  • Gather your documentation. This includes vaccination records (translated to English if necessary), proof of tuberculosis treatment (if applicable), and a government-issued ID for yourself and any children immigrating with you.
  • Need an interpreter? Consider bringing a friend or family member 18 years old or older to interpret. We can also provide interpretation services for a small fee.
  • Dress comfortably in loose clothing for easier examination during the physical.
  • Prepare for blood work. Drink plenty of water beforehand and avoid urinating for 4 hours to ensure a good sample.

First Visit: Paperwork and Initial Tests

Your first visit will involve completing some initial paperwork and undergoing a few preliminary tests. To help expedite this process, please arrive 30 minutes early. We’ll review your application details and ensure you have all the required documentation, such as a government-issued ID, vaccination records, and medical records (see the full list below). Our clinical staff will then collect blood samples for age-specific tests. All this information will be documented on your Form I-693, a key part of your immigration application.

Here’s What to Bring to Your First Visit

  • Government-issued ID (with photo) for yourself and any children immigrating with you
  • Vaccination records (translated to English if necessary)
  • Any medical records, including hospital discharge summaries, specialist reports, or records of past treatments (if applicable)
  • Prior chest X-rays or other relevant diagnostic tests (if applicable)
  • Mental health records (if applicable)
  • USCIS correspondence you’ve received (if applicable)
  • Payment (cash or credit card)

Second Visit: Civil Surgeon Exam

During your second visit, scheduled about a week after your initial appointment, you’ll meet with a USCIS-approved doctor, known as a civil surgeon. These specialists conduct immigration physicals to ensure you meet the health requirements for permanent residency. The civil surgeon will begin by thoroughly reviewing your medical history. Be prepared to discuss any past health conditions, current medications, and any surgeries you’ve had.

Following this discussion, the civil surgeon will perform a comprehensive physical exam to assess your overall health, checking your heart, lungs, reflexes, and general well-being. Based on your medical history and the exam, the civil surgeon may recommend additional vaccinations or tests, such as chest X-rays, blood tests for specific diseases, or mental health evaluations. Once everything is complete, including any recommended tests and vaccinations, the civil surgeon will finalize your Form I-693. You’ll then receive a sealed envelope containing the official I-693 for immigration purposes, along with a copy for your records.

What to Expect at Your Immigration Physical Exam

Your doctor will order a chest X-ray and blood tests. Your physical and mental disposition will be assessed through X-rays, blood tests, and diagnostic observation. In addition to your Form I-693, bring your medical and vaccination records for the doctor. If you don’t have proof of your vaccinations, you can request a blood test to prove immunization.
