Immigration Examinations at AFC Hudson Urgent Care:
AFC Hudson Urgent Care is proud to offer immigration medical examinations (A.K.A: Green Card examinations) to immigration applicants seeking US citizenship in and surrounding Hudson, NH and northern MA. The examination will be done by an M.D. (known as a Civil Surgeon) who is authorized and designated by the U.S. Citizenship and Immigration Services (USCIS) to perform this examination.
What to expect:
A minimum of two visits are required for each immigration examination.
Visit 1: This visit will be scheduled with a Medical Assistant ~ 5 days before your 2nd visit to begin the laboratory testing and gather required documents and review your completed I-693 form.
Visit 2: The second visit will be with your Civil Surgeon to complete your examination and obtain any necessary immunizations. This is typically scheduled ~ 1 week after the 1st visit.
Additional visits may be necessary depending on findings during your examination &/or laboratory test results. We ask that you arrive ~ 30 minutes before your appointment to complete necessary paperwork. Your Civil Surgeon will review your past medical history and conduct your physical examination. At the completion of your examination, you will be guided as to any immunizations you will need to obtain, any tests or further medical care needed.
Once all required data is collected, you will be notified that your documents are ready for you to pick up and be processed with your immigration attorney.
What you need to bring:
- Form I-693, Report of Medical Examination and Vaccination Record. You may complete this form online or you may fill one out at AFC Hudson. Please capitalize all your responses and ONLY use a pen with black ink.
- Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
- Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease, and COVID-19) all translated in English.
- Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious diseases.
- Prior chest x-rays and/or other diagnostic tests.
- Payment via credit or debit card or bank check. Cash and personal checks are NOT accepted.
Costs for the Immigration Examination:
Cost Per Applicant: (1/2 collected at the 1st visit and ½ collected at the 2nd visit)
- Adults (15 years +): $525
- Children (14 years or under): $450
All Testing (completed by Quest Diagnostics) and Examinations include:
- Physical Examination by your Civil Surgeon
- Gonorrhea Test (via urine or swab collection)
- RPR (Syphilis) Test (blood test)
- QuantiFERON-TB Gold Test (blood test)
- Titers for Hep B, MMR, Varicella (blood test)

- Chest X-Ray: $115
- TDAP: $110
- Flu (September 15- March 30) – $40
- MMR: Not administered at AFC, referred to PCP or local pharmacy
- HEP A: Not administered at AFC, referred to PCP or local pharmacy
- HEP B: Not administered at AFC, referred to PCP or local pharmacy
- Varicella: Not administered at AFC, referred to PCP or local pharmacy
- Certified Medical Translation Services: $125
Immigration examination documentation not submitted to USCIS/local Immigration attorney within 60 days of the date it was completed and signed by your Civil Surgeon needing to be repeated fee:
- Physical with paperwork- $250 (New appointment is required.)
